I respectfully apply for a license to engage in the business or occupation of pruning, treating, or removing trees within the City of Mulvane, Kansas. The license fee shall be $50.00 annually and shall be valid for twelve (12) months from the date of issue. A certificate showing that liability insurance of the minimum amount of $50,000.00 for bodily injury and $100,000.00 property damage must accompany this application before a license can be issued.
